Life Care Management: Elder Care Coordinators
The staff of Kennedy West provides life care management services that include having an Elder Care Coordinator who works intimately with you and your family to answer questions and help guide you in finding the right resources for both in-home and community services needed for your loved one. The Elder Care Coordinator will give you a recommended plan that will focus on your loved one's quality of life, and can also coordinate the care needed to meet the needs of your loved one. Your family's plan will be re- evaluated and updated, by the Elder Care Coordinator, to meet individual goal needed by your loved one.
